Job description Job responsibilities

Work with primary care team and patients to resolve medication queries. Supporting medication reviews and medication reconciliation Assisting with repeat prescribing and ensuring safe dispensing. Monitoring adherence to practice, local and national prescribing policies and guidance. Supporting clinical pharmacists in delivering clinical pharmacy services. Providing advice and information to patients and other healthcare professionals. Undertake structured medication reviews. Triaging and managing common ailments. Conducting medication audits. Providing information to patients on medicines and treatment. Supporting patients with medication changes and managing their medication. Signposting patients to relevant support services. Prescribing Audits in line with CQC and QOF targets

Supporting the implementation of safe and efficient repeat prescribing systems.
