Job description Job responsibilities Clinical Pharmacist (ARRS Funded) Part Time / Maternity CoverWe are seeking an enthusiastic and motivated Clinical Pharmacist to join our multidisciplinary primary care team at The Hollies Medical Centre. The successful candidate will work alongside GPs, nurses and administrative staff to deliver high-quality patient-centred care, with a focus on medicines optimisation, chronic disease management and safe prescribing.Key responsibilities include undertaking clinical medication reviews, managing complex polypharmacy, supporting repeat prescribing systems, responding to medication queries, medicines reconciliation following hospital discharge, and supporting care home medication reviews. The role also involves contributing to prescribing audits, quality improvement initiatives, QOF and enhanced services, and promoting safe and cost-effective prescribing in line with NICE and local guidance.Applicants should be a qualified pharmacist registered with the GPhC, ideally with independent prescribing qualifications and experience working in primary care or general practice. Excellent communication, clinical decision-making and team-working skills are essential.
