Job description Job responsibilities Core duties include: Performing medication reviews, advising patients on prescriptions, and optimising prescribing Liaising with GPs across all sites and working closely with the prescriptions team Supporting the duty GP with urgent prescriptions enquiries Carrying out medication reviews, including new patient medication reviews and structured medication reviews Medication reconciliation and outpatient scripts Supporting with audits, QIP projects, and CQC-related audit work Audit for use of safe prescribing and high risk drugs Working with Prescription Clerks (based at Kiln Farm) and supporting them with queries and medicines management Further duties may include, but are not limited to: Conducting patient consultations as an independent prescriber Assisting with medicines compliance and quality audits Liaising with community pharmacies regarding stock and supplies Supporting Prescription Clerks in initiating or amending medications from secondary care Providing clinical management/support for the prescription team Assisting in the development of prescribing processes and protocols Acting as the first point of contact for prescription-related queries Supporting our local care homes with queries regarding prescriptions
