Job description Job responsibilities The PCN is looking to employ a Clinical Pharmacist under the Additional Roles Reimbursement Scheme, the PCN must ensure that each Clinical Pharmacist has the following key responsibilities in relation to delivering health services as outlined in the Network Contract Direct Enhanced Service Contract Specification 2025/2026 PCN Requirements.
Clinical Pharmacists work as integral members of the multidisciplinary team (MDT), delivering patient-centred care with a focus on medicines optimisation, long-term condition management, and improving prescribing quality across the PCN.Patient Care & Long-Term Condition ManagementClinically assess, treat, and manage patients in a patient-facing role.Manage a caseload of complex patients, particularly those with chronic conditions (e.g. diabetes, hypertension, COPD, asthma, frailty) and polypharmacy.Deliver long-term condition clinics and support achievement of EPC targets for relevant disease areas.Undertake structured medication reviews to optimise treatment, improve adherence, and support shared decision-making in line with professional frameworks.Medicines Optimisation & Prescribing QualityLead on medicines optimisation, ensuring safe, evidence-based, and cost-effective prescribing.Deliver medication optimisation projects, including clinical audit, cost analysis, and quality improvement initiatives aligned with NICE and local guidance.Provide expert advice on prescribing, medication monitoring, and high-risk medicines.Act as the PCN lead for medicines-related queries and prescribing systems.Leadership, Collaboration & IntegrationProvide leadership on patient-centred medicines optimisation across the PCN.Work collaboratively with general practice, community pharmacy, hospital teams, and wider health and social care services.Build strong professional relationships across the PCN and act as a key point of contact for medicines-related matters.Support integration of services to improve patient outcomes and continuity of care.Education, Training & DevelopmentDeliver teaching and training in specialist clinical areas to MDT colleagues and wider staff.Contribute to clinical supervision, service development, and continuous improvement of the role.Keep up to date with national and local guidance (e.g. NICE, CQC) and ensure implementation in practice.Service Delivery & GovernanceSupport delivery of enhanced services and PCN service requirements.Contribute to clinical governance, including audits, incident reporting, complaints management, and learning from events.Participate in mandatory training, appraisals, and ongoing professional development.Support effective medicines management processes and workload across practices.Patient Support & NavigationRespond to medicines-related queries from patients and staff.Proactively signpost patients to appropriate healthcare professionals and services.Address public health and social care needs as part of holistic patient care.There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels.
