You will be joining an exceptional team working together to help our patients to regain independence with their medication regimes and stay healthy when they need it most. This pharmacist will provide a competent and efficient pharmaceutical service to hospital inpatient units and patients at home to meet the changing operational demands of the service. Ideally you will have completed, or be on your way towards finishing a clinical diploma and have experience in a hospital pharmacy setting. We will consider applications from experienced pharmacists from any sector.We are looking for a candidate who: demonstrates excellent communication and problem-solving skills shows initiative and adaptability in a dynamic healthcare environment will work with the wider pharmacy team to support safe and timely discharge of patients and manage a caseload of referred patients requiring an intervention at home. is flexible, organised and committed to delivering patient-centred care can prioritise workload effectively in response to changing patient needsThis position sits within an experienced, ambitious and motivated pharmacy team. The post-holder will receive appropriate training with plenty of scope to develop in the role, and enjoy a high level of job satisfaction. There is the expectation to qualify and use independent prescribing skills to facilitate excellent patient focused care while promoting medicines safety, admission avoidance and discharge facilitation.
