The post holder will provide specialist clinical pharmacy services to cancer inpatients at Singleton Hospital. Working within the multidisciplinary team, they will support the safe, effective and timely use of medicines throughout the cancer pathway and contribute to the development of cancer pharmacy services. Key responsibilities include delivering specialist clinical pharmacy care; providing expert advice on oncology, haematology and supportive care medicines; supporting the safe prescribing, verification and governance of systemic anti-cancer therapy (SACT); participating in ward rounds, MDTs and discharge planning; developing policies, guidelines and shared care protocols; investigating medication incidents; undertaking audit, research and quality improvement; providing education and clinical leadership to pharmacy and multidisciplinary colleagues; supporting service development and innovative models of care; and maintaining professional competence through continuing professional development. Previous experience in cancer services is desirable but not essential. The successful candidate will receive structured training, including specialist oncology education, supervised practice and completion of SACT competencies where required. You will also contribute to the ongoing development of the cancer pharmacy service through participation in clinical audit, service evaluation, quality improvement initiatives, education and training, medicines governance, and the monitoring and analysis of medicines expenditure, helping to ensure safe, effective and efficient use of medicines across the Directorate.
