Job description Job responsibilities Key ResponsibilitiesMedicines Optimisation and PrescribingConduct face-to-face and remote structured medication reviews.Support safe, effective, and cost-efficient prescribing.Process and manage prescription requests and medication queries.Review and implement medication safety alerts, MHRA guidance, and prescribing updates.Undertake medicines reconciliation following hospital discharge and clinic correspondence.Manage repeat prescribing systems to ensure safe and appropriate medication use.Chronic Disease ManagementProvide clinical support and patient reviews for long-term conditions, including:DiabetesHypertensionAsthmaCOPDThis may include medication optimisation, treatment escalation, patient education, monitoring, and adherence support as required.Blood Test and Medication MonitoringReview blood test results related to medication monitoring.Contact patients by telephone regarding abnormal or outstanding results.Arrange and monitor required follow-up investigations in line with clinical protocols.Ensure compliance with medication monitoring requirements and clinical safety standards.Minor Ailments ServiceAssess, diagnose, and manage patients presenting with minor illnesses and common ailments within your scope of practice.Provide treatment advice, prescribing where appropriate, and onward referrals when necessary.Quality, Safety and GovernanceSupport the practice in achieving and maintaining CQC compliance and quality standards.Assist with clinical audits, quality improvement projects, and medicines management initiatives.Identify and mitigate prescribing risks to improve patient safety.Contribute to practice policies, protocols, and medicines-related governance activities.Essential RequirementsRegistered Pharmacist with the General Pharmaceutical Council (GPhC).Independent Prescriber qualification (or working towards obtaining one).Experience in Primary Care, General Practice, PCN, or Community Pharmacy.Strong knowledge of medicines optimisation, prescribing, and long-term condition management.Excellent communication, consultation, and organisational skills.Ability to manage workload independently while working collaboratively within a multidisciplinary team.Desirable RequirementsPrevious experience supporting CQC inspections and evidence gathering.Experience with EMIS Web and NHS clinical systems.Experience running clinics for chronic disease management.Advanced clinical assessment and consultation skills.What We OfferSupportive and friendly multidisciplinary team.Opportunities for professional development and clinical training.Competitive salary based on experience.NHS Pension Scheme (where applicable).Opportunity to make a meaningful impact on patient care and service development.Summary of Main DutiesThe Advanced Pharmacist Practitioner will be responsible for:Medication reviews.Medication safety alerts and medicines optimisation.Supporting CQC compliance and quality improvement.Chronic disease management (Diabetes, Hypertension, Asthma and COPD as required).Prescriptions and repeat prescribing management.Blood test monitoring and management, including patient telephone consultations.Minor ailments assessment and management.Supporting safe and effective patient care across the practice.
