Job description Job responsibilities Undertake structured medication reviews (SMRs) for patients with complex polypharmacy, long-term conditions, frailty, or high-risk medicines.Support safe and effective repeat prescribing systems within practices.Provide advice and support on medicines management, prescribing queries, and cost-effective prescribing.Contribute to the management of long-term conditions by optimising medicines use and supporting self-care.Conduct medicines reconciliation following hospital discharge and ensure appropriate follow-up.Liaise with community pharmacists, hospitals, and other providers to ensure continuity of care.Review prescribing in line with local and national guidelines to ensure safe, effective, and efficient use of NHS resources.Support practices to meet QOF, IIF, DES, and other contractual requirements through high-quality prescribing.Provide support and advice to GPs, nurses, and ARRS staff on medicines-related issues.Attend PCN and practice meetings, contributing to service development.Engage with population health management initiatives and help address health inequalities through medicines optimisation.
