Band 7 Intermediate Care/Hospital Pharmacist

Job description Job responsibilities The main duties will include medicines reconciliation, medication review, professional checking, clinical advice, patient counselling, self - administration assessment and coaching, ensuring medication supplies from community pharmacy. You may also be involved in incident reporting and investigation and training for other healthcare professionals. You will be able to attend MDT meetings and Quality improvement meetings. The role will also include similar work at our 2 hospitals as well as on call duties. On call will cover enquiries from the whole trust and may occasionally involve supply from one of the trust pharmacies.
