Job description Job responsibilities Job purposewe are looking for a motivated and enthusiastic clinical pharmacist to join our friendly and forward thinking team at shipdham surgery as part of the mid norfolk primary care network this is an exciting opportunity to play a key role in delivering high quality patient care within a supportive multidisciplinary teamthe post holder will work within their clinical competencies to improve the quality safety and effectiveness of care for patients you will work closely with gp partners nurses and the wider team to support safe prescribing optimise medicines use and help manage patient demand across the practiceMain duties and responsibilitiesprovide patient facing medication reviews with a focus on long term conditions polypharmacy and frailtymanage prescription requests and make clinical decisions including prescribing where appropriatesupport patients with medicines queries and provide advice on minor ailmentswork with care homes to improve medicines safety and carry out medication reviewssupport safe prescribing through audit medicines optimisation and quality improvement workassist with hospital discharge medicines reconciliation and reduce medicines related harmcontribute to antibiotic stewardship and infection prevention initiativeswork collaboratively with community pharmacy hospital teams and other healthcare professionalssupport the development of efficient systems for prescribing and medicines management within the practicecommunication and teamworkbuild strong working relationships within the practice and across the wider healthcare systemcommunicate effectively with patients colleagues and external organisationswork as part of a team recognising when to seek support from senior cliniciansdelivering a quality serviceorganise and prioritise workload effectivelymaintain up to date clinical knowledge and professional registrationcontribute to training education and continuous improvement within the team
