Job description Job responsibilities The Clinical Pharmacist will: Undertake structured medication reviews and medicines optimisation for patients with long-term conditions, frailty, or high-risk prescribing. Support the repeat prescribing process and medicines reconciliation post-discharge or transfer of care. Provide specialist clinical advice on all aspects of medicines management to GPs, nurses, and other healthcare professionals. Contribute to the achievement of PCN service requirements, QOF, and other quality improvement initiatives. Support the delivery of national and local prescribing guidelines and formulary adherence. Assist in auditing, reporting and implementing safe prescribing practices in line with CQC and NHS England standards. Lead in the reduction of prescribing of antibiotics, over the counter medicines and high cost medicines. Where qualified, act as an Independent Prescriber to manage patients directly within own scope of competence. Be responsible for the clinical supervision of the Pharmacy Technicians
