Job Description

Job description Job responsibilities

Conduct Structured Medication Reviews (SMRs) for patients with single or multiple medical conditions, identifying and prioritising patients according to clinical risk and contract requirements. Review ongoing medicine needs, monitoring requirements, and opportunities for medicines optimisation, making recommendations to clinicians or implementing changes as appropriate. Involve patients in decisions about their prescribed medicines and support adherence in line with NICE guidelines. Work with care home staff to improve the safety of medicines ordering and administration. Provide patient-facing and telephone clinics for those with questions, queries, and concerns about their medicines in the practice. Respond to medicine-related enquiries from GPs, practice staff, community pharmacies, and other healthcare teams, providing advice, solutions, and follow-up to patients to monitor the effect of any changes. Review medicines associated with unplanned hospital admissions and readmissions through audits and individual patient reviews, implementing changes to reduce the prescribing of these medicines to high-risk patient groups. Identify cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches. This might include risks that are patient-related, medicine-related, or both. Contribute pharmaceutical expertise to the development and implementation of new services that have medicinal components, including treatment pathways and patient information leaflets. Analyse, interpret and present medicines data to highlight issues and risks to support decision making. Undertake simple audits of prescribing in areas directed by the GPs, provide feedback on the results, and implement changes in conjunction with the practice team. Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance. Provide education and training to the primary healthcare team on therapeutics and medicines optimisation. Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved.

To support public health campaigns and provide specialist knowledge on all public health programmes available to the public.
