Job description Job responsibilities Main Duties of the RoleThe successful applicant will undertake a varied clinical role including:Delivering Structured Medication Reviews.Managing patients with long-term conditions.Medicines optimisation and deprescribing.Polypharmacy and frailty reviews.Supporting chronic disease management clinics.Independent prescribing (where qualified).Providing expert medicines advice to clinicians and patients.Responding to medication-related clinical queries.Medicines reconciliation following hospital discharge.Supporting prescribing governance and medicines safety.Leading and participating in clinical audit and Quality Improvement projects.Supporting prescribing efficiency and evidence-based practice.Working collaboratively with GPs, Pharmacy Technicians, Nurses and the wider multidisciplinary team.Supporting the delivery of vaccination and immunisation services, including routine childhood immunisations where appropriately trained.
