Job description Job responsibilities Patient ServicesManagement of medicines following hospital discharge and action medication changes Repeat prescribing and annual medication reviews Structured Medication Reviews (SMRs) and clinical medication reviews Telephone medicines support for patients Patient-facing clinics including remote consultations Management of common/minor/self-limiting ailments within scope of practice Identify patients at risk of unplanned hospital admissions Medicines safety including MHRA alerts Support public health campaigns such as Flu and COVID vaccination programmes Signposting to community pharmacy, GPs and other healthcare professionalsMedicines Management & Quality ImprovementSupport effective medicines management across WFHP Medicines Quality Improvement programmes (QOF, IIF, CEIF) Clinical audit and service improvement Implementation of local and national prescribing guidelines Use open prescribing data to improve prescribing standards Support Care Quality Commission (CQC) complianceEducation and TrainingProvide training and support to prescription teams and pharmacy staff Support training of pharmacy technicians and foundation clinical pharmacists Provide medicines information to staff and patients Participate in monthly CPD sessions
