Job description Job responsibilities Job ResponsibilitiesClinical Practice and Medicines Optimisation Undertake patient-facing Structured Medication Reviews (SMRs). Optimise medicines use for patients with long-term conditions and complex medication regimens. Provide medication reviews following hospital discharge and care transitions. Support repeat prescribing processes in line with practice policies. Respond to medication-related queries from patients and healthcare professionals. Promote shared decision-making and medicines adherence. Support management of long-term conditions including diabetes, asthma, COPD and hypertension within scope of practice and competence and under the supervision of GPs. Medicines Safety and Governance Support the implementation of medicines optimisation and prescribing safety initiatives within the practice. Contribute to medicines-related audits, quality improvement projects and service evaluations. Assist in monitoring prescribing performance against local and national guidance. Support the review and implementation of MHRA alerts, NICE guidance and safety communications. Participate in significant event reviews and learning activities relating to medicines management. Contribute to the maintenance of safe prescribing systems and processes. Escalate medicines-related risks and concerns appropriately. Multidisciplinary Team Working Work collaboratively with GPs, nurses, pharmacy technicians and administrative teams to improve patient care. Provide medicines advice to members of the multidisciplinary team. Liaise with community pharmacies, secondary care providers and ICB medicines optimisation teams where required. Support implementation of local and national medicines optimisation initiatives. Contribute to achieving practice and PCN quality improvement objectives. Clinical Governance Maintain accurate and legally compliant clinical records. Work in accordance with GPhC standards, NHS policies and practice procedures. Maintain professional competence through continuing professional development. Support medicines-related governance and quality assurance activities. Education and Training Maintain professional registration and meet all CPD requirements. Participate in mandatory training programmes.
