Job description Job responsibilities The post holder will play a key role in delivering safe, effective, and evidence based medicines management services within primary care.Key Responsibilities: Provide clinical pharmacy support to patients and the wider primary care team Independently prescribe and manage medicines for patients within agreed clinical areas Manage long term conditions in line with local and national guidelines Support medicines optimisation, deprescribing, and cost effective prescribing Support Pharmacy Technicians with medicine reconciliations following hospital discharge and outpatient clinic letters Contribute to prescribing audits, QOF, and quality improvement projects Work collaboratively with GPs, nurses, pharmacy technicians, and administrative teams Support training and development within the pharmacy and wider practice team
