Job description Job responsibilities The post holder will support patients in achieving optimal outcomes from their medicines by implementing agreed protocols for medication switches, improving repeat prescribing processes within General Practice, and promoting repeat dispensing and online ordering. The role will also focus on minimising clinical risk and reducing medicines waste.Working as part of a multidisciplinary team in a patient-facing role, the Clinical Pharmacist will clinically assess and treat patients, using advanced knowledge and experience to manage a defined cohort of patients.They will contribute to the delivery and development of pharmacy-led medicines optimisation services across General Practice and the wider multidisciplinary team, ensuring practice remains aligned with current guidance and best evidence.The role includes leading on medicines optimisation within a specialist area, supporting improved safety, outcomes, and value for money.The post holder will also lead and contribute to evaluation, audit, and service development to ensure high-quality delivery and continuous improvement.All work will be carried out in line with medicines optimisation principles, national guidelines, and relevant evidence-based practice.
