Job description Job responsibilities Job PurposeThe Clinical Pharmacist will work as an integral member of the multidisciplinary team at Market Weighton Surgery, providing expert clinical pharmacy services to improve patient outcomes, optimise medicines use, support long-term condition management, and contribute to the delivery of high-quality primary care services.The post holder will undertake structured medication reviews, manage patients with complex polypharmacy, support prescribing quality initiatives, and provide specialist pharmaceutical advice to clinicians and patients. Key ResponsibilitiesClinical Practice Conduct patient-facing and remote structured medication reviews. Manage and optimise medicines for patients with long-term conditions. Support patients with complex medication needs, including: Older adults and frail patients. Care home residents. Patients with multiple long-term conditions. Patients with learning disabilities and autism. Prescribe safely within individual scope of practice as an Independent Prescriber. Identify, manage, and reduce risks associated with medicines. Undertake medication reviews following hospital discharge and ensure medicines reconciliation. Provide advice and support regarding common illnesses and self-care. Signpost patients to the most appropriate healthcare professional when required. Medicines Optimisation Promote safe, cost-effective, evidence-based prescribing. Support implementation of local and national prescribing guidance. Monitor prescribing data and identify opportunities for quality improvement. Lead on repeat prescribing processes and medication monitoring systems. Respond to MHRA alerts, drug safety updates, and national guidance. Assist in maintaining and reviewing practice formularies. Patient Safety and Quality Improvement Identify patients at risk from inappropriate prescribing. Participate in clinical audits and quality improvement projects. Support achievement of Quality and Outcomes Framework (QOF) and locally commissioned targets. Contribute to medicines-related significant event reviews and learning. Support antimicrobial stewardship initiatives. Multidisciplinary Working Work closely with GPs, nurses, healthcare assistants, community pharmacists, care homes, and secondary care teams. Attend multidisciplinary meetings where appropriate. Provide specialist medicines advice to the wider practice team. Support integrated working across local healthcare providers and YHP networks. Education and Training Deliver medicines-related education and training to practice staff. Support the development of prescribing knowledge across the wider clinical team. Maintain professional competence through continuous professional development (CPD). Participate in appraisal and mandatory training. Leadership and Service Development Contribute to the development and implementation of pharmacy-led services. Support the Practice and YHP in delivering strategic objectives relating to medicines optimisation. Analyse and interpret prescribing and medicines management data. Promote innovation and continuous improvement in service delivery. Professional ResponsibilitiesThe post holder will: Maintain registration with the General Pharmaceutical Council (GPhC). Adhere to GPhC Standards for Pharmacy Professionals. Work within professional competencies and scope of practice. Maintain patient confidentiality and comply with GDPR requirements.
