Job description Job responsibilities Through a varied rotational programme, to work within multidisciplinary healthcare teams to provide wardbased clinical pharmacy services, taking responsibility for clinical screening, medicines reconciliation, and optimisation of prescriptions within own scope of competence. To deliver medicines optimisation and the safe transfer of care around medicines, including supporting accurate medicines reconciliation at admission and discharge and effective communication with primary and secondary care providers. To undertake a postgraduate diploma in clinical pharmacy, subject to funding being available. To undertake necessary continuing professional development in line with General Pharmaceutical Council standards and to practise in accordance with the GPhC Standards of Conduct, Ethics and Performance. To provide clinical pharmacy support to inpatient and outpatient dispensaries and to undertake the role of Responsible Pharmacist when required. To participate in the provision of medicines information and advisory services to patients and their relatives, medical and nursing staff, and other healthcare professionals and organisations, including patient counselling and support to enable safe and effective use of medicines. To actively contribute to medicines safety by identifying, reporting, and learning from medication incidents, near misses, and risks, in line with Trust medicines governance and incident reporting systems. To provide professional supervision to pharmacy technical and support staff. To contribute to the training and development of trainee and undergraduate pharmacists and pharmacy technicians. To participate in clinical audit and quality improvement initiatives aimed at assuring and improving the quality, safety, and effectiveness of prescribing and pharmacy practice. To contribute to the practice and development of quality governance throughout the pharmacy department. To undertake identified clinical and professional training and actively participate in the staff appraisal process. To contribute to the delivery and development of clinical pharmacy services across the Trust. To provide training and education to other healthcare professionals, supporting safe and effective medicines use. To utilise electronic prescribing, medicines administration, and other digital systems effectively to support safe, accurate, and auditable pharmacy practice. GENERAL RESPONSIBILITIES To undertake any training (managerial and professional) identified through the staff appraisal process. To participate in oncall duties, weekend and Bank Holiday working, early and late shifts, in accordance with agreed rotas and service needs. In the event of a disruptive incident, all Trust employees may be required to attend work if they are fit, well, and able to do so, in line with business continuity and major incident plans. To maintain confidentiality at all times relating to patients, staff, and Trust information, in accordance with information governance and data protection requirements. To actively participate in the Staff Appraisal Process. To be familiar with and comply with relevant operational, personnel, health and safety, and Trust policies and procedures, including Fire, COSHH, No Smoking and Alcohol policies. To maintain a safe working environment by adhering to local infection control policies and promoting safe professional practice. To contribute to an open and learning culture by supporting the Duty of Candour and raising concerns where patient safety, professional standards, or wellbeing may be compromised. To work in a way that promotes equality, diversity, and inclusion, ensuring that pharmaceutical care is responsive to individual patient needs.
