Job description Job responsibilities Support the delivery of medicines optimisation initiatives within the practice. Assist with identifying and resolving medication-related problems. Promote evidence-based prescribing and adherence to local and national guidance. Support medication safety audits and quality improvement projects.Clinical Support Manage medication queries from patients, GPs, nurses, and other healthcare professionals. Conduct medication reconciliation following hospital discharge and outpatient appointments. Review repeat prescribing requests and make recommendations in line with practice protocols. Assist in monitoring patients receiving high-risk medicines. Support management of long-term conditionsPrescribing Support Assist with reviewing prescribing against local formularies and prescribing guidelines. Support cost-effective prescribing initiatives. Contribute to identifying opportunities for deprescribing where clinically appropriate. Escalate complex clinical cases to senior pharmacists or GPs as required.Patient Care Provide advice to patients regarding their medicines, including administration, side effects, and adherence. Support shared decision-making and patient understanding of treatment plans. Respond appropriately to patient concerns and medication-related enquiries.Multidisciplinary Working Work collaboratively with GPs, nurses, healthcare assistants, pharmacy technicians, and administrative staff. Participate in multidisciplinary team meetings where appropriate. Develop effective working relationships with community pharmacies, secondary care, and other healthcare providers.Governance and Quality Maintain accurate and contemporaneous clinical records. Ensure compliance with relevant legislation, professional standards, and practice policies. Participate in clinical audits, service evaluations, and quality improvement activities. Contribute to the reporting and investigation of medication incidents and significant events.
