Job description Job responsibilities Band 6JOB SUMMARY To provide pharmacy support to allocated wards and departments and provide cover for the clinical pharmacists in their absence. Provide medicines reconciliation, discharge planning and services across the primary/secondary care interface. To attend multidisciplinary team meetings for their identified wards and provide input into pharmaceutical care planning. To provide support to the dispensary to ensure safe provision of medicines to the wards, departments and individual service users.KEY RESULT AREAS: Participate in all aspects of the pharmacy service as required by the Chief Pharmacist Participate in ward and clinical pharmacy services for identified teams / units including the community units. Support the provision of medicines reconciliation in line with NICE and Trust guidance. Provide support to the dispensary at Fieldhead, Wakefield. Assist with the accreditation process and training of technicians undergoing the Accredited Technician Checking scheme. Participate in the dispensing and supply of medication to patients. Supervise the work of supporting staff. Provide training to pre-registration technicians and pharmacists regarding the service provided and support the Senior Technician with the training of staff in dispensary-based matters. Participate in the medicines management clinical audit programme. Act as the responsible pharmacist when in charge of the pharmacy as the allocated dispensary pharmacist. GENERAL To observe the principles of Clinical Governance in general and in particular be able to demonstrate participation in Continuing Professional Development in line with General Pharmaceutical Council requirements. This will include attendance at appropriate training courses. To participate in the Trusts Appraisal system. The post is based at Fieldhead Hospital, but the post holder may be required to provide cover for other wards / teams which are not based in the Wakefield locality as dictated by service need. Excess milage will be paid in these instances. This job description will be reviewed regularly in the light of changing service requirements and any such changes will be discussed with the postholder. To participate in the provision of a safe and effective specialist clinical pharmacy service to designated wards within SWYPFT.2. To assist Principal Pharmacist / Senior Clinical Pharmacist in the overall provision of pharmacy services to units/teams within SWYPFT.3. To deputise for Principal Pharmacist / Senior Clinical Pharmacist as appropriate.ROLE OF YOUR DEPARTMENT1. Maintain and develop efficient and cost effective systems for procurement, compounding, preparation and distribution of medicinal products, which reflect the needs of individual patients/patient groups as determined by the Trust.2. To ensure compliance with the current law relating to medicines and medicinal products.3. To consider and adopt as appropriate Department of Health advice and guidance relating to the pharmaceutical services provided.4. To provide professional advice to relevant health care professionals and patients regarding the safe, cost-effective use of medicines and medical products.5. To identify and ensure the operation of quality control mechanisms that meet formal controls required by the medicines agencies.6. Identify and address clinical risk issues by applying principles of clinical governance, Medicines Management and controls assurance.RESPONSIBILIITES (Budgetary/Supervisory)1. Supervisory to liaise with the Principal Pharmacist / Senior Clinical Pharmacist in co-ordinating the activities of: Clinical supervision: Band 6 (Pharmacists) Band 5 (Technicians) Co-ordinate activities of: Ward Based Technician(s) Assistant(s)This job description is an outline of the main duties of the post. The postholder will be required to undertake other duties commensurate with the grade as directed.The content of this post will be reviewed in consultation with the postholder when necessary and in line with the service developments. KEY RESULT AREAS: To provide a specialist clinical service to wards and departments of SWYPFT in accordance with Trust and department policies. This will include: Taking drug histories on admission. Reviewing medication to ensure safe and effective use. The formulation of individual care plans. Reviewing and updating care plans depending on response, results of investigations and adverse effects. Therapeutic drug monitoring for specific drugs. Educate patients about their drug therapy. Develop evidence based treatment protocols for use either within the directorate or Trustwide where appropriate and work with other pharmacy colleagues to comment on and support implementation of policies and procedures. Facilitation of compliance aids and self-medication schemes where appropriate in order to increase patient safety, improve concordance and increase quality of life in this unique patient group.To undertake therapeutic drug monitoring for relevant medications in order to optimise efficacy and tolerability (examples include clozapine, lithium, anticonvulsants etc) To undertake clinical audit and practice research within the Pharmacy Department where appropriate. To record clinical interventions and complete query record forms in line with departmental policy. To collate all Trust pharmacist clinical interventions and report on trends and outcomes where requested as appropriate by the Principal Pharmacist / Senior Clinical Pharmacist. Promote evidence based cost-effective prescribing. To provide professional and legal clinical supervision in the dispensary and to participate in the checking and dispensing of prescriptions (including compliance aids) when required as directed by the Technical Services Manager. To develop and maintain links with other professionals, such as dieticians, nursing and medical colleagues regarding service developments and improvements. To recommend service changes and implement them where appropriate. This may include changing paperwork, changing storage or delivery arrangements etc. To support nursing staff and Medicines Management pharmacy technician by providing clinical pharmacy advice to the patients self-administration scheme and to undertake ongoing evaluations of individual outcomes of patient self-administration. To participate in the appropriate reporting process: to report, record and monitor medicines related incidents or complaints and to explore ways of minimising risk of future re-occurrence or risk to staff. To undertake the dispensing of clinical trials in accordance with the dispensing protocol, ICH GCP guidelines and European Directives. To provide clinical training for Pharmacy Undergraduates, Pre-registration Pharmacists, Postgraduate Pharmacists and Technicians. To participate in the training of medical and nursing staff as required. To participate in the out of hours rotas. To comply with the legal and other requirements related to the purchase, supply, use, safe custody and destruction of drugs within Pharmacy and in all other areas in the Trust. To comply with the Health and Safety at Work Act, Data Protection Act and COSHH legislations. To participate in Continuing Professional Development as per department policy and national guidelines from the General Pharmaceutical Council.
