Develop clinical pharmacy programs according to policies and regulationsReview records of patients to determine the appropriateness of medication therapyEvaluate patients condition to ensure all issues are being treatedIdentify untreated health problems and refer patients to appropriate physiciansDevelop effective medication plans that minimize the risk of adverse side-effectsConsult on dosages, medication substances etc.Advise on the correct administration of drugsAssess the results of pharmaceutical treatmentsCollaborate with healthcare professionals to ensure optimal patient careKeep accurate documentation of medication plans and patient progress
