Job description Job responsibilities The Role:As an onsite Clinical Pharmacist, you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and pharmacy technicians.You will help resolve medication-related queries, work as part of a multidisciplinary team, improve patient outcomes and support the improvement of access to healthcare.Main duties of the jobAs a clinical pharmacist you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and GP's.Resolve medication-related queriesPolypharmacy medication reviews and SMRsFace to Face medication reviewsHelp to utilise skill mixImprove patient outcomesEnsure better access to healthcareSupport practice staff with prescription and medication queriesMedicine reconciliation and review post-dischargeConduct medical related audits and work on service improvement with the practiceProvide expert advice on medications to other clinicians and patientsDeliver clinical leadership on medicines optimisation and quality improvementEnsure appropriate and efficient use of medicationEducate patients on medication and disease preventionAn understanding of QOF, CQC and local incentive scheme workstreamCreate and follow clinical pharmacy programme according to practice policyJob Requirements:A minimum of 12 months experience of working within general practice/primary careA relevant degree/qualification to show you are registered Clinical Pharmacist with GPHC registrationIndependent prescribing qualificationExperience with EMIS or System 1Completed the CPPE Primary Care pathway or close to completionAble to commute to onsite location / live within close proximity of practiceExcellent communication and interpersonal skillsBenefits for employees:GPhC fees and DBS Applications paid5 weeks of annual leave plus bank holidays (Pro rata as and when required)Sick pay (In line with company policy)Health & wellbeing programme with the following free services, confidential counselling, digital gym, high street stores discountsAn established network of over 200 pharmacists nationally and dedicated senior pharmacist support availableCompany equipment e.g. laptop / smartcard readerFlexibilityPension schemeThe Pharmacist Network are committed to providing a diverse and inclusive workforce, and welcome applications from candidates regardless of sex, race, disability, age, sexual orientation, gender reassignment, religion or belief, marital status, or pregnancy and maternity.
