Job description Job responsibilities Key Responsibilities Work as part of a multidisciplinary team in a patient-facing role to assess and treat patients using expert knowledge of medicines Manage chronic diseases and conduct structured medication reviews for patients with complex polypharmacy Develop and implement medicines management plans in line with local and national guidance Maximise cost-effective prescribing and improve the quality of patient care Provide specialist advice on medicines, addressing public health and social care needs while helping to tackle health inequalities Build strong relationships with pharmacy professionals across PCNs and the wider healthcare system Contribute to shared care protocols, clinical research, and liaison with specialist pharmacists, community pharmacies, and anticoagulation services
