Job description Job responsibilities My role helps deliver patient care by supporting Independent Prescribers to develop their knowledge, learning and expertise to enhance their practice and confidence.The post holder will be a qualified Community Pharmacist Independent Prescriber and will establish and facilitate a bi-monthly Community of Practice (CoP) meeting in each of the three Integrated Care Areas (ICA) in Cornwall. The CoP Facilitator(s) can cover one or more areas.Key Duties and Responsibilities:To work on behalf of Cornwall Training Hub, in partnership with Kernow Health CIC, to plan, coordinate and establish a Community of Practice (CoP) group for Community Pharmacy Independent Prescribers across the three Integrated Care Areas: West Cornwall, Central Cornwall and North and East Cornwall.To facilitate at least one structured evening event in each Integrated Care Area on a bi-monthly basis.To create a supportive, collaborative environment that promotes shared learning, peer support, professional development and encourages reflective practice discussions in line with professional and governance standards.To actively promote the Community of Practice to prescribers within Community Pharmacy with the support of Kernow Health CICs Community Pharmacy Project Manager.To build strong professional relationships to encourage ongoing attendance and engagement.To identify learning needs within each locality and tailor sessions, accordingly, supported by Kernow Health CICs Community Pharmacy Project Manager.With the support of Kernow Health CICs Community Pharmacy Project Manager organise and manage the logistics of evening events/group, including agenda setting, venue coordination and communication with participants,Develop sessional content which is aligned with local and national needs and priorities.Facilitate interactive discussions, learning and knowledge sharing activities.Monitor participation and engagement levels across the ICAs and with the support of Kernow Health CICs Community Pharmacy Project Manager implement strategies to increase participation as necessary.Gather feedback from participants to evaluate the effectiveness of the sessions, adapting and improving the structure of the CoP based on the feedback and emerging needs.Ensure all discussions and activities adhere to professional, ethical and information governance.You will be expected to carry out any other duties that may reasonably be required in line with your main duties, as directed by your line manager.
