Job description Job responsibilities Clinical Practice

Undertake structured medication reviews (SMRs) for patients with complex medication needs, polypharmacy, frailty, learning disabilities, and long-term conditions.

Provide clinical medication reviews to improve patient outcomes and reduce medicines-related harm.

Support the management of patients with chronic diseases through evidence-based prescribing and medicines optimisation.

Assess, diagnose, and manage patients within their scope of competence and qualifications.

Develop and implement personalised care and support plans.

Identify and address medication adherence issues.

Provide advice and support regarding medication use, side effects, and treatment options.

Monitor high-risk medicines in accordance with local and national guidance.

Support care home residents through regular medication reviews and medicines optimisation activities.

Medicines Optimisation

Promote safe, effective, and cost-efficient prescribing across the practice.

Lead on medicines reconciliation following hospital discharge and outpatient appointments.

Review repeat prescribing processes to improve safety and efficiency.

Respond to prescribing queries from clinicians, patients, carers, and external healthcare providers.

Support the implementation of local and national prescribing guidance.

Identify opportunities to improve prescribing quality and reduce waste.

Assist with formulary compliance and prescribing incentive schemes.

Prescribing Safety and Clinical Governance

Undertake prescribing audits and quality improvement projects.

Monitor and action Medicines and Healthcare products Regulatory Agency (MHRA) alerts and safety updates.

Support the practice in responding to national patient safety alerts.

Identify patients at risk from medication-related problems and implement appropriate interventions.

Contribute to clinical governance, risk management, and patient safety initiatives.

Maintain accurate and contemporaneous clinical records.

Long-Term Condition Management

Support the management of patients with conditions such as:

Hypertension

Diabetes

Asthma

COPD

Cardiovascular disease

Chronic kidney disease

Mental health conditions

Undertake medication reviews and treatment optimisation in line with national guidance and local pathways.

Multidisciplinary Team Working

Work collaboratively with GPs, nurses, healthcare assistants, care coordinators, social prescribers, and administrative staff.

Participate in multidisciplinary team meetings.

Liaise with community pharmacies, hospital pharmacists, and specialist services.

Provide expert medicines advice to the wider practice team.

Support training and education of clinical and non-clinical staff.

Quality and Outcomes Framework (QOF) and Enhanced Services

Support the achievement of QOF targets through medicines optimisation and clinical interventions.

Contribute to enhanced services and local commissioning initiatives.

Identify and recall patients requiring review in line with contractual requirements.

Education and Training

Maintain professional registration and continuing professional development.

Participate in appraisal, personal development planning, and mandatory training.

Contribute to the education and supervision of trainees, students, and other healthcare professionals.

Support the educational ethos of the practice as a GP Training Practice.
