Job description Job responsibilities Provide clinical medication reviews, including SMRs, for patients with longterm conditions and polypharmacy.Manage acute and repeat prescription requests, ensuring safe and effective prescribing.Support longterm condition management and contribute to medicines optimisation across the practice.Run pharmacistled clinics (face to face and remote) for medicationrelated issues.Action medicinesrelated tasks from total triage and Patchs online consultations.Lead on medicines safety, audits, MHRA alerts, and antimicrobial stewardship.Contribute to QOF, IIF and PCN medicinesrelated objectives.Provide medicines advice to the multidisciplinary team and support training of learners.
