Job description Job responsibilities Review and develop systems and processes for monitoring prescribing, procurement, storage dispensing, distribution and administration of medicines which reduce the risk of medication errors. Support the development of education and training programmes to enable multidisciplinary teams to discharge their responsibilities relating to the prescription, procurement, supply and administration of medicines. Design and develop systems and processes alongside clinical colleagues to mitigate the risk of incidents recurring in the Trust Review safety alerts and national safety documents working alongside colleagues to prepare appropriate plans to deliver compliance. Lead serious untoward incident reviews where appropriate and provide a quality assurance function for incident reviews involving medicines. Support multidisciplinary teams to investigate incidents to enable the root cause to be understood and appropriate plans to be established to mitigate future risks. Promote the reporting and monitoring of medication errors within the pharmacy and wider Trust. Analyse medication error data from the Trust and from Primary Care and provide information and prepare reports to support risk reduction strategies across the Trust and at the interface between primary and secondary care. Identify and review incident trends and work with other healthcare professionals to find and implement solutions to prevent medication errors. Lead and support others to implement and learn using Patient Safety Incident Response Framework to ensure the development and implementation of robust action plans in response to medication errors to mitigate the risk of re-occurrence. Demonstrate innovation to identify and promote best practice aimed at mitigating the risk associated with medicines use. Work with the Trust E-prescribe Team and Pharmacy IT Team to find IT solutions to minimise medication errors. To provide highly specialist advice and support to the Trust leads overseeing litigation and claims in the Trust related to medicine safety and where necessary attend MDT meetings /Coroners courts. This may include representing the Trust at coroners court to explain medication related policies and decisions. Lead and develop others to lead the Trust response to national alerts and guidance relating to medication safety and ensure compliance with deadlines. Undertake medication related risk assessments and supporting others to prepare assessment where required. Support with the management of the Risk Register on behalf of the Pharmacy Department and the Trust for corporate medicines management issues and ensure that action is taken or obstacles to progress are escalated in line with the Trust governance structures. Coordinate feedback to staff relating to medication errors and related solutions to all disciplines to minimise future risk and support organisational learning. Be responsible for reviewing risk management reports relating to pharmacy or medicines to identify the need for process change or education.
