PCN Clinical Pharmacist

Job description Job responsibilities Key Responsibilities: Provide clinical input into the safe and effective use of medicines within the PCN. Undertake structured medication reviews (Level 3) to optimise patient outcomes. Support the wider clinical team with prescribing queries and guidance. Identify unmet pharmaceutical care needs using risk stratification tools. Promote rational and evidence-based prescribing in line with NICE guidance. Participate in clinical audits, quality improvement initiatives, and national/local incentive schemes. Contribute to the training and development of other healthcare professionals.Please see attached Job description for more information.
