Job description Job responsibilities The successful candidate will provide expert pharmaceutical care, supporting patients across a range of clinical settings. Main duties of the role will include: Conducting patient-centred structured medication reviews to optimise medicines use and improve health outcomes. Providing higher-level clinical administration and medicines management support to ensure safe and effective prescribing practices. Managing and supporting the ongoing care of patients with long-term conditions, using your independent prescribing qualification where appropriate. Identifying and addressing medicines-related issues, improving patient safety and reducing unnecessary hospital admissions. Working collaboratively as part of a multidisciplinary team (MDT), contributing your specialist knowledge to support holistic patient care. Supporting the delivery of national and local clinical priorities, including prescribing quality initiatives and service improvement projects.
