Job description Job responsibilities Medication Review and OptimisationRespond to pharmacy queries to include clarifying doses and or products and give appropriate alternatives when availability issues occurDiscuss medication issues with patients as needed and appropriate by telephone for example adverse effects interactions overdose inadvertent ingestion OTC remedies queries from practice dispensary and queries from care homesDiscuss specific patient and prescribing issues such as complicated regimes polypharmacy compliance difficulties multiple adverse effects and medication reduction regimes with other clinicians as necessaryFace to face and video consultations with patients may includeDiscuss medication issues as needed and as appropriateCarry out medication reviewReview and continue repeat medications for new patients who have just registered with the practice and suggest GP medication review where appropriateCare Home Medication ReviewsSupport GPs with management of care home residentsUndertake clinical medication reviews with patients with multimorbidity and polypharmacy and implement own prescribing changes if an independent prescriber and order relevant monitoring testsWork with care home staff to improve safety of medicines ordering and administrationLong Term Condition ClinicsAccording to experience and training undertake chronic disease reviews and medicine optimisation according to practice guidelinesHypertension lifestyle advice and medication initiation and optimisationDiabetes joint clinic with practice nurse with referral to GP community diabetes nurse specialist or secondary care diabetes advice lineCHD medicines optimisation with referral to GP where necessaryAsthma and COPD medicines optimisation with referral to GP where necessaryChronic pain management responding to patient or GP requests for reviewMedicines ReconciliationReview secondary care requests for new medication as communicated by discharge summaries outpatient letters and other correspondence raise any queries with relevant GP and discuss how to respond when inappropriate prescribing requests ariseDeal with anticoagulant start and stop requests from secondary careUnplanned Admission PreventionDevise and implement practice searches to identify cohorts of patients most likely to be at risk of unplanned admission and readmission from polypharmacyWork with practice team to put in place changes to reduce the prescribing of these medicines to high risk patient groupsMedicines Information and EducationMonitor and inform colleagues about ongoing prescribing issues including new national and local guidelines new products requested by secondary care manufacturing and supply problems prescribing restrictions contraindications and prescribing errorsAdvise on cost effective prescribing and prescribing budget issuesPrescribing Systems and PoliciesAssist in identifying patients in need of medication review and developing systems to meet this needWork with GPs and practice teams to develop and implement safe and efficient prescribing policies and strategies for CAM Medical Primary Care Network to maximise efficiency and reduce wastageSuggest and design audits in relation to prescribing targets implementation of locality policies and the Quality Outcomes Framework QOFWork with GPs and practice managers on delivering targets for local prescribing quality schemesMonitor CAM Medical Primary Care Network prescribing performance and advise on changes to practice prescribing as appropriateOversight and audit of high risk drug monitoringMedicines SafetyIdentify national and local policy and guidance that affects patient safety through the use of medicines including MHRA alerts product withdrawals and emerging evidence from clinical trialsLiaison with Community and Hospital PharmaciesSupport integration of CAM Medical Primary Care Network with community and hospital pharmacies and proactively manage patients at risk of medicine related problems on discharge to provide continuity of careRunning Reports and AuditsConduct clinical audits of prescribing in designated areas as directed by the clinical lead and or GPsProvide feedback on findingsCollaborate with the practice team to implement necessary changesAny Other Duties as Reasonably RequiredUndertake any other appropriate duties as may reasonably be
