Job description Job responsibilities Key ResponsibilitiesEnsure compliance with NHS guidelines, local policies and professional standards.Develop and embed safe, efficient prescribing processes within the practice.Contribute to clinical audits, quality improvement initiatives and service developmentSupport safer prescribing of high risk medications e.g. DMARDS, anticoagulants, opioids etc.Conduct medication reviewsPrescribe medications safely and appropriately as an independent prescriber.Support the management of long-term conditions such as diabetes, hypertension, CKD, asthma, COPD and CVD.Review and optimise patient medications to improve safety, adherence and clinical outcomes.Design, implement and refine protocols, SOPs and medicine workflows.Identify and reduce prescribing risk, polypharmacy issues and medication errors.Provide patients with advice and education regarding medicines and treatment plans.Maintain accurate and up to date clinical records.Work collaboratively.Participate in continuing professional development and mandatory training.Authorising repeat and acute medications.Dealing with patient medicine queries
Adding and removing medications at the request of secondary care.
