Job description Job responsibilities Medicines Optimisation & Medication ManagementProvide expert medicines management support within GP practicesUndertake medication reconciliation following hospital discharge and outpatient correspondenceSupport safe, effective and cost-efficient prescribing in line with NICE, AWMSG and Health Board guidanceManage and support repeat prescribing systemsRespond to prescribing and medication queries from patients, clinicians, community pharmacies and secondary careClinical Medication ReviewsUndertake structured medication reviews (SMRs) and annual medication reviewsFocus on patients with:PolypharmacyFrailtyLong-term conditionsHigh-risk medicinesSupport shared decision-making, deprescribing where appropriate, and improved adherenceAnticoagulation & High-Risk MedicinesManage patients prescribed DOACs / NOACs, including:Dose appropriatenessMonitoringAnnual reviewsProvide INR dosing and warfarin management in line with local protocolsSupport safe use of high-risk medicines (e.g. opioids, lithium, methotrexate)Shared Care & Interface WorkingManage and support shared care agreementsReview and action medication-related hospital correspondenceAct as an interface between primary care, secondary care and community pharmacyCare Home, Frailty & Complex CareSupport medicines optimisation for care home residentsUndertake frailty-focused medication reviewsReduce inappropriate polypharmacy and medicines-related harmWork closely with GPs, care home staff and MDT colleaguesQuality Improvement & Prescribing ProjectsContribute to and lead Quality Improvement (QI) projectsDeliver Prescribing Management Scheme (PMS) projects, audits and service evaluationsUse prescribing data to support service improvement and patient safetyEducation, Training & Training Practice SupportAs a training practice, Amman Tawe Partnership values education and workforce development.The post holder will:Support the education of GP trainees, medical students and Physician Associate (PA) studentsPromote safe, evidence-based prescribing and medicines optimisation principlesContribute to a positive learning environment across the practiceSPECIFIC RESPONSIBILITIESWork autonomously with a high level of clinical decision-makingLead complex medication reviews (polypharmacy, frailty, high-risk medicines)Take a lead role in QI initiatives and PMS projectsProvide structured support, teaching and mentorship for GP trainees, medical students and PA studentsAct as a clinical resource and role model within the MDTSupport service development across the partnershipGovernance & Professional ResponsibilitiesMaintain accurate and timely clinical recordsWork within local and national governance frameworksParticipate in audit, service development and significant event analysisMaintain GPhC registration and CPD requirements
