Job description Job responsibilities As a Pharmacy Technician, you will work under the supervision of the Lead Clinical Pharmacist to support the delivery of safe, effective and patient-centred pharmaceutical care across the practice.Your responsibilities will include:
Supporting medicines optimisation and prescribing safety initiatives.
Undertaking medication reviews and medicines reconciliation.
Managing repeat prescribing processes and responding to medication queries.
Supporting high-risk medicines monitoring, including Direct Oral Anticoagulant (DOAC) optimisation.
Completing clinical searches, audits and quality improvement projects.
Supporting delivery of QOF, Enhanced Services and medicines optimisation initiatives.
Working collaboratively with GPs, Clinical Pharmacists, Nurses, Community Pharmacies and Secondary Care providers.
Supporting care home medicines optimisation and transfer of care processes.
Contributing to service development and continuous improvement across the practice.
Supporting the education and development of trainee Pharmacy Technicians, Foundation Trainee Pharmacists and other learners where appropriate.
