Pharmacy technician

Job description Job responsibilities The Pharmacy Technician will support the delivery of safe, effective and patient-centred medicines optimisation across the practice, working as part of a multidisciplinary team.The role includes supporting patients to use their medicines effectively, assisting with medication reviews and medicines reconciliation following hospital discharge or care transitions, and helping to coordinate monitoring requirements for structured medication reviews. The postholder will contribute to improving prescribing safety, reducing medication waste, and supporting antimicrobial stewardship.You will work closely with GPs, pharmacists, care homes and community pharmacies to ensure continuity of care, including managing shared care processes and supporting patients with long-term conditions.

The role also involves improving prescribing systems and processes, promoting efficient ordering and use of the Electronic Prescription Service (EPS), and supporting colleagues with medicines-related queries. There is a strong focus on collaboration, service improvement, and contributing to wider health outcomes for the practice population.
