Job description Job responsibilities The Pharmacy Technician will play a key role in supporting the safe, effective, and efficient management of medicines within the practice as part of the multidisciplinary team. The post holder will be responsible for maintaining accurate patient medication records, processing and generating prescriptions, and reconciling medicines following hospital discharge or outpatient appointments.The role involves acting as a central point of contact for prescription-related queries from patients and healthcare professionals, and liaising with colleagues across primary, community, and secondary care to ensure continuity of care. The Pharmacy Technician will support the implementation and monitoring of repeat prescribing systems, medicines optimisation processes, and regulatory requirements.In addition, the post holder will contribute to clinical audits, support quality improvement initiatives, and promote cost-effective prescribing in line with local and national guidance. Although primarily administrative with remote patient contact, the role offers the opportunity to develop into a more patient-facing position supporting long-term condition management and preventative care.Key Duties and Responsibilities1. To update and maintain accurate patient medication records on the practices clinical computer systems, including advice given and action taken.2. To advise the primary health care team on the safe and secure handling of controlled drugs and other medicines, ensuring compliance with medicines legislation.3. Generate prescriptions from requests from patients.4. Reconciliation of medicines from outpatient and discharge letters.5. Liaise with hospital, community and primary care colleagues to ensure correct medicines are continued following transfer of care.6. Supporting practice staff to deliver on the CQC inspections, MRHA alerts, DES contract, QIPP agenda, QOF and locally commissioned enhanced services7. Point of contact for the practice for prescription related queries for healthcare professionals and patients8. Implementing and monitoring the practices adherence to a repeat prescription policy
