Job description Job responsibilities The post holder will work as part of a multi-disciplinary team to provide expertise in clinical medicines management, patient-facing structured medication reviews, manage long term conditions, manage medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, and acute prescription requests, while addressing the health and social care needs of the practice's patients.
Main duties:
Conduct surgery and telephone consultations with patients with minor illnesses
Patient-facing medication reviews of patients with polypharmacy especially for older people and people with multiple co-morbidities.
Provide advice and support for patients with self-limiting conditions
Reconcile medicines following patients discharge from hospital or clinic appointments
Review medications for newly registering patients
Implement and embed a safe, effective and evidence based repeat prescribing system
Practice as an independent prescriber working within scope of practice
Work as part of a multi-disciplinary team to manage repeat prescription authorisations and reauthorisation, acute prescription requests and systems for safer prescribing
Provide leadership to the practice on quality improvement and clinical audit as well as managing some aspects of the Quality and Outcomes Framework, and assisting with QOF and local performance targets
