Patient Services:

To provide a medication review and query services to patients via the telephone or face to face clinics as appropriate in the practice.See (where appropriate) patient with single or multiple medical problems, including patients with long term conditions, where medicine optimisation is required.Provide pharmacist support to clinicians attending care home or to attend themselves as appropriate. Work with care home staff to improve safety of medicines ordering and administration.Managing caseload of patients with common /minor/self-limiting ailments while working within a scope or practice and limits of competence. Signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate.Please see the attached Job Description for further information.
