Job description Job responsibilities Brief summary below - please see attached Job Description for full detailsJob responsibilitiesThe post holder is an experienced clinical pharmacist with previous experience of working in Primary Care, who acting within their professional boundaries and as part of the wider clinical team, will develop and manage medicine management services within the practice.The post holder will lead the multi-disciplinary pharmacy team, to meet practice service requirements for medicines management.The postholder will manage systems and be part of the workforce relating to repeat prescribing, authorisations and reauthorisation, acute prescription request, management of medicines on transfer of care and systems for safer prescribing.The post holder will lead processes and perform medication review of patients with polypharmacy especially for older people, people in residential care homes and those with multiple co-morbidities.The post holder will provide leadership on medicines optimisation, leading on local ICB projects and schemes of work, quality improvement and clinical audit.See attached Job Description for full details;Leadership of the practice pharmacy teamLine managing the team to ensure an effective high quality service is providedManaging the practice pharmacy serviceLead and contribute to all national and local projects, initiatives contracts and schemes.Managing change for medicines management.Provision of clinical services.Running own long term conditions workWorking as part of the urgent care teamManaging patients referred by other members of the wider practice teamMedication reviews
