Senior Specialist Pharmacist (Oncology/Haematology)

Job description Job responsibilities To develop and maintain a pharmacy service that is proactive and responsive to the needs of patients and service users Provide a comprehensive clinical pharmaceutical service including prescription monitoring and supply, provision of drug information, evidence-based practice and patient counselling, including participation in ward rounds. Provide medicines advice and information to improve prescribing within the multidisciplinary team (MDT), using evidence-based medicine guidelines. To contribute to the development, implementation and monitoring of policies, procedures, guidelines and standards aimed at improving medicines management and patient safety To contribute to the vision and development of Pharmacy Clinical Services and associated team development. To promote, develop and contribute to practice research. To establish links with HCA and external Pharmacists to benchmark medicines use and promote good networking practices. To undertake membership/representation within relevant internal and external clinical forums
