Job description Job responsibilities Why you should join our Team The LTHT Renal service provides high quality care for the population of Leeds and the surrounding areas as one of the largest regional, specialised services for pre-dialysis, haemodialysis and peritoneal dialysis. The Leeds Renal Department is also the regional centre for kidney transplantation and performs around 200 transplants per year. Our renal pharmacy team now support the prescribing of all post-discharge renal transplant immunosuppression medication for LTHT patients. We are involved with the care of patients in both the inpatient and outpatient setting and are currently setting up pharmacist led renal transplant clinics. Our specialist pharmacists and technicians are integral to the safe and effective use of medicines. You will contribute to developments and innovative strategies to improve the medicines service available to all our patients and other users. Leeds Teaching Hospitals has an international reputation for excellence in specialist care, research and academic training. Our Clinical Pharmacy Teams cover a wide variety of medical & surgical specialities from Paediatrics to Elderly Medicine. What you will bring to the role Provide a specialist clinical pharmacist service to patients and the multidisciplinary team within a given specialty. Ensure medicines are used appropriately, safely, and cost effectively in accordance with Trust policy, standard operating procedures, and medicines legislation. Monitors drug use within their clinical area. This includes recording of significant clinical interventions, risk management, and some basic financial aspects. Supervise, along with other members of their multidisciplinary team, the development and implementation of treatment protocols, and guidelines for use of drugs within the clinical area. Responsible for training pharmacists, and other health care professionals, on aspects of drug use within the clinical area. Provides minimum of at least one 4 hour session of service cover in the dispensary or aseptic dispensary per week. To act as the Responsible Pharmacist in line with local procedures.
