Provide a comprehensive clinical pharmacy service within the Emergency and Integrated Care (EIC) Directorate, ensuring the safe, effective and cost-efficient use of medicines through prescription reviews, medicines optimisation, patient counselling, discharge planning and participation in multidisciplinary ward rounds. Support and develop medicines management initiatives by applying specialist pharmaceutical knowledge, clinical reasoning and governance principles to improve patient outcomes and minimise medication-related risks. Communicate effectively with patients and wider healthcare professionals, making appropriate referrals and providing expert medicines advice when required. Contribute to leadership, service management and quality improvement agenda by supervising and mentoring junior staff, promoting evidence-based practice, participating in audit and research activities, supporting clinical pathway development and monitoring service performance. Maintain professional standards through ongoing education and training, adherence to legal and ethical frameworks, commitment to equality, diversity and inclusion, and active participation in training and continuous service improvement initiatives.
