To provide complex medicines information to patients, carers, and healthcare professionals, ensuring advice on dosing, side effects, and indications is clear, accurate, and tailored to the recipients level of understanding. To act as a role model in training and education, supporting the development of pharmacists, technicians, assistants, and other healthcare staff, while promoting and facilitating Continuing Professional Development across the department. To contribute to the development of clinical guidelines and protocols, supporting the preparation of reports for healthcare committees and ensuring best practice in medicines use across the Trust. To ensure safe medicines management practices, including reporting drug errors, undertaking controlled drug checks, complying with legal and regulatory requirements, and adhering to professional and Trust policies. To support audit and research activities, identifying opportunities for pharmacy practice improvement, monitoring adherence to prescribing guidelines, and contributing to evidence-based enhancements in patient care. To collaborate with multidisciplinary and primary care teams, communicating key information regarding medicines, treatment plans, and follow-up to ensure continuity of care across hospital and community settings.
