Job description Job responsibilities Duties and Responsibilities of the PostClinical RoleAs a prescriber, to prescribe safe, evidence-based, cost-effective, effective and appropriate medication and individualised patient care as defined by current legislative framework.Promote and help compliance with the Dorset Formulary.Refer patients directly to other services/agencies as appropriate.Work with the Multi-Disciplinary Teams within the practice to promote integrated and seamless pathways of care.Be prepared to embrace templates, IT and all computer services to update knowledge.Develop bespoke integrated pharmaceutical care plans for individual patients.Engage with care homes to improve prescribing to residents.Work with nursing, medical staff, health care assistant colleagues to ensure that Quality Outcomes Frameworks are being delivered.Liaise with hospital pharmacy teams and specialists where appropriate especially when completing medicines reconciliation from discharges.Manage patients with more complex long-term conditions. This will involve direct patient contact in the pharmacists own clinic sessions.Manage repeat prescribing requests, and work to rationalise and improve repeat prescribing systems.Offer a holistic approach to the use and understanding of medicines by patients that includes step-up and step-down management required in Long Term Conditions and End of Life.Facilitate access to medicines (e.g. sorting out secondary care referrals for specials, unusual formulations etc.).Interpret national and local commissioning decisions as they relate to medicines.Identify opportunities for savings and efficiencies where that does not conflict with the interests of individual patients.

Professional RolePromote evidence-based practice through use of the latest research-based guidelines.Support excellence and innovation in care through clinical audit/quality improvement/ clinical research where appropriate.Monitor the effectiveness of their clinical practice through quality assurance strategies such as the use of audit and peer review.Maintain their professional registration.Participate in continuing professional development to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained.Record accurate consultation data in patient records in accordance with the latest guidance and other pertinent standards.Actively take part in mandatory and further training as requested.Annual practice appraisal review.Adhere to practice guidelines re confidentiality.Follow all practice Health and Safety guidelines and policies.
