Job description Job responsibilities Role Accountabilities Clinical Assessment and PrescribingContribute to the delivery of safe, effective and evidence-based clinical assessment, diagnosis and prescribing within scope of practice.Apply advanced clinical knowledge and judgement to determine appropriate treatment plans, including medicines optimisation and deprescribing where required.Manage a defined caseload, prioritising patients based on clinical need and service requirements.Medicines Optimisation and SafetyContribute to safe and effective use of medicines, ensuring prescribing aligns with clinical guidelines and best practice.Identify and respond to medication-related risks, escalating complex or high-risk situations appropriately.Support safe prescribing practices and contribute to minimising medicines-related harm.Patient Care and EngagementProvide patients with clear advice, education and support to enable informed decision-making and adherence to treatment.Use communication and influencing skills to promote behaviour change and optimise therapeutic outcomes.Multidisciplinary and Service CollaborationWork collaboratively with multidisciplinary teams, contributing to coordinated patient care and clinical decision-making.Support integration of prescribing activity across services and care pathways.Build effective working relationships with colleagues, patients and stakeholders to support service delivery.Governance, Compliance and QualityContribute to the application of clinical governance, including audit, risk management, safeguarding and incident reporting.Maintain accurate, timely and compliant clinical records in line with legal and professional standards.Support continuous quality improvement and service development initiatives.Professional Practice and DevelopmentMaintain professional competence, including prescribing practice, through supervision, reflective practice and continuing professional development.Keep up to date with relevant clinical guidance, legislation and best practice, applying these in day-to-day work.Contribute to audit, training and small-scale service improvement activity where appropriate.
