Job description Job responsibilities The practice pharmacy technician will support all practice staff, including GPs, nurses, admin staff and receptionists in providing an efficient, effective service to our patients.Dealing with patient medication requests, preparing prescriptions and to answer patient queries face to face and by telephone.Dealing with patient medication requests, preparing prescriptions and to answer patient queries face to face and by telephone.Reconcile the medications of patients whose care is transferred back to primary care in a timely and effective manner liaising with patients and other providers to ensure patients receive appropriate medication on dischargeWork with the GPs and Practice management team to develop safe, effective, and efficient systems for the repeat prescribing of medicationWork with GPS to implement changes to medicines in line with MHRA alerts, product withdrawal and other local or national guidanceManage requests to prescribe medication from specialists in line locally agreed Shared Care guidelines and escalate to GP for more complex patients.Proactively identify patients who require blood tests or GP review before their next prescription is issued to support safe prescribing. This includes but is not limited to high risk drugs.Carry out medication housekeeping e.g. dose optimisation and medicines synchronisationTo respond to medication queries from patients and staff in a professional manner referring to the appropriate clinician in accordance with protocols and personal and professional boundaries.To support the achievement of the practices prescribing quality and outcomes framework (QOF) targets.Provide professional telephone and written advice and support to patients and their carersParticipate in audit activities to optimise medicines safety and the use of prescribing resourceAny other duties as required by the Practice within the boundaries of training, experience and competence.
