Job description Job responsibilities The Pharmacy Technician will be undertaking a variety of key responsibilities in delivering health services:1. Assist the Prescription Clerks with medicines reconciliation to ensure accurate transfer of medications across health settings when receiving discharge summaries and clinic letters from secondary care.2. Conduct medicines optimisation tasks including effective medicines counselling, supporting medication reviews and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.3. Maintain strong working connections with local pharmacies to ensure continued patient safety between settings and prompt query resolution.4. Provide specialist expertise, where competent, to address both the public health and social care needs of patients including lifestyle advice, service information, and help in tackling local health inequalities.5. Maintain a central role in the clinical aspects of shared care protocols and local formulary and liaise with hospital specialists for more complex patients.6. Assist in the delivery of medicines optimisation and management incentive schemes and patient safety audits.7. Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings. This will be achieved through undertaking clinical audits, supporting quality improvement measures and contributing to the Quality and Outcomes Framework (QOF) and enhanced services.8. Undertake medication safety tasks such as high-risk medication monitoring and eclipse radar safety alerts.9. Provide support and guidance to the prescription clerks through supervision and query resolution.10. Work collaboratively with the existing Pharmacy Technician to develop prescription team related policies and procedures and maximise patient outcomes.11. Attend a formal appraisal with your manager at least every 12 months. Once a performance/training objective has been set; progress will be reviewed on a regular basis so that new objectives can be agreed.

There is potential scope for the Pharmacy Technician to engage in Pharmacy Technician-led medicines optimisation clinics following appropriate training and under the guidance of the prescribing lead GP.
