Job description Job responsibilities Liaising with local pharmacies and hospitals regarding prescribing queries, optimum therapy, to improve prescribing safety, quality, cost effectivenessProcess and update patient medication from hospital discharge and clinic lettersTo action and process medication alerts, raising awareness within the practiceTo help plan, develop and support the introduction of new working processes within the practice to optimise the quality of prescribing.To oversee patients on shared care drugs, ensuring recalls and protocols in place to ensure safety and appropriate use of medicines and their managementTo support the Clinical Pharmacist to provide medication reviews, patient information leaflets and cost effectiveness of prescribing.To ensure maintenance of accurate patient records and ensure all relevant information is documented in the patients medical records in a timely mannerTo help plan, develop and support the introduction of new working processes within the practice to optimise the quality of prescribing
