Job description Job responsibilities You should refer to the supporting documents for a full Job Description outlining the core responsibilities of the role.Key responsibilities include: Supporting medicines optimisation and safe prescribing across the Partnership Undertaking medicines reconciliation following hospital discharge and other care transitions Supporting repeat prescribing processes and medication reviews Assisting with structured medication reviews and high-risk drug monitoring within your competency Responding to pharmacy-related patient enquiries and supporting medicines advice services Completing prescribing audits and contributing to quality improvement initiatives Working collaboratively with Clinical Pharmacists, GPs, prescribing teams, and pharmacy administrators Maintaining accurate clinical records and ensuring compliance with prescribing policies and procedures Using digital systems and innovative technologies to improve patient access and prescribing efficiency Contributing to the ongoing development of the Pharmacy Hub and new models of care You will thrive if you enjoy medicines optimisation, collaborative working, embracing new technologies, supporting patients and clinicians, and contributing to the continual improvement of pharmacy services.
